Alberta Ability Network (an initiative of Cerebral Palsy Alberta) is a collaborative of stakeholders, people with lived experience and those who support them, health practitioners, academia, community members, and government that strives to:

As Albertans transition to ADAP, we want to hear your experiences with the transition. This form is completely anonymous, and we will make every attempt to further remove any identifiable information that you may have shared. 
We are NOT caseworkers or advocates that can help you with the transition, you will not receive a response to what you enter here, even if you enter contact information.
Experiences shared with us will be used to inform advocacy efforts, may be shared (anonymized) with policy and law makers, and may be used on social media or websites (anonymized).
